**Penn State Health Community Medical Group** **Location:** US:PA:Reading **Work Type:** Full Time **FTE:** 0\.9 **Shift:** Vaired **Hours:** Varied **JOB SUMMARY:** Under supervision, performs general receptionist, care coordination duties\. Duties include: answering and screening telephone calls, scheduling appointments; Receiving incoming mail/ courier deliveries for clinicians including consults, labs results, insurance correspondence including pending authorizations, grievances, denials and approvals\. Attaching documents to appropriate patient charts\. Communication with clinicians as appropriate to review, prep, proofread FMLA forms, DME forms, physical forms, etc\.; Maintains an ongoing relationship with local agencies, DME suppliers, etc\.; Receives and reviews reporting from after\-hours phone triage/answering service\. Forwards reports to appropriate staff for follow\-up; Composes LMN for equipment, supplies, and services; Facilitates patient resource needs for diapers, formula, nutrition supplements, home nursing through Medical Assistance; Acts as practice primary contact for home nursing agencies; Facilitates communication between parents, schools, insurance companies, agencies as needed; Provides clerical support to office manager and clinicians as needed; Assists with incoming faxes and distributes to staff for follow\-up and/or attaches to patient charts Priority of this position is to provide coordination of care for patients using excellent communications skills\. **MINIMUM QUALIFICATIONS:** High School Diploma or equivalent required **KNOWLEDGE, SKILLS, & ABILITIES:** Ability to speak, read, write and understand the English language Ability to lift 25 pounds Occasional kneeling, pushing/pulling and reaching Ability to operate a personal computer and general office equipment Ability to deal courteously with patients Ability to maintain effective working relationship with other employees **PREFERRED QUALIFICATIONS:** Prior experience working in a medical office preferred This job description is a general outline of duties performed and is not to be misconstrued as encompassing all duties performed within the position\.
