Overview:Full Time - ACT RN (Community nursing)Tower Behavioral
Health is a BRAND NEW, state of the art, psych-safe facility that
partners with the ACT team to support the community with behavioral
health needs.Our team is looking for dedicated and compassionate
RN's to join our Assertive Community Treatment (ACT) team.Our
Registered Nurses are active members of an integrated treatment

nursing services to patients in the communityProvides support and
psychoeducation about mental illness diagnoses and
symptomsIndependent living skills and illness management

planningInformation and/or referrals to resources in the community
for increased support and stabilizationSkill-development training
for independent living skills, daily living skills & social
skillsEDUCATION/EXPERIENCE/SKILL REQUIREMENTS:Graduated from an
accredited school of nursing.Current Registered Nurse (RN) license
in the State of PennsylvaniaCurrent CPR certification are
required.Previous psych nursing experience is highly
preferred.Qualifications:EDUCATION/LICENSURE/EXPERIENCE/SKILL
REQUIREMENTSMust have graduated from an accredited school of
nursing.Must possess a current Registered Nurse (RN) license as
required by the state of employment or be eligible to practice
nursing in accordance with state requirementsOne year of nursing
experience; previous psychiatric, mental, behavioral, or substance
abuse nursing experience a plusCPR and de-escalation and restraint
certification required (training available upon hire and offered by
facility).First aid may be required based on state or facility
requirements.AHRNResponsibilities:ESSENTIAL FUNCTIONS:Provide and
coordinate care by assessing physical and behavioral health needs
of patient, develop and implement nursing care plans, maintain
medical records and educate patients and their families about
various physical and behavioral health conditions.Provide nursing
care and services within the standards and scope of nursing
practice as defined within the state Nurse Practice Act
regulations, policies, procedures and protocols, as well as
facility and corporate policies and procedures.Ensure patient's
status is assessed on an ongoing basis and pertinent information
gathered is documented.Collaborate with others to ensure that all
patients' physical, biopsychosocial, age, developmental and
cultural needs are met and when they are not met, acknowledge and
work to resolve customer complaints.Notify physician of significant
changes in status such as difficulties with detox, medical
emergency or change in medical status regarding response to
medications, etc.Ensure nursing treatment plans are specific to
assessed needs and initial treatment plan is completed upon
admission.Demonstrate knowledge and utilization of universal
precautions in providing direct patient care.Accurately administer
medications per physician orders and document accurate count of
controlled substance medication administration on required
documents.Responsible for completing admission and discharge
processed efficiently and accurately as directed.Exhibit
therapeutic rapport with patients by maintaining professional
boundaries, being respectful to personal space, giving patient
direct attention and maintaining rational detachment.Communicate
therapeutically and professionally with patients, visitors,
coworkers and others involved in patient treatment in an effective
manner.Responsible for coordinating continuum of therapeutic goals
for assigned clients and their families ensuring optimum
utilization of resources, service delivery, and compliance with
external agencies and referral sources requirements.Provide initial
treatment planning for patient based on assessment and coordinate
any additional services needed and revise as necessary.Maintain
documentation of services provided to each patient according to ACT
regulations and professional standards, revising as
necessary.Maintain a caseload as assigned.
