Job Description Job Description General Practice Physician Opportunity This role is for a General Practice Physician who will deliver primary care in skilled nursing facilities, concentrating on the management of intricate patient cases within a post-acute care environment. The position requires collaboration with multidisciplinary teams, spearheading care coordination efforts, and mentoring nurse practitioners to enhance patient outcomes. In this full-time role (5 days a week), you will manage complex patient cases in a skilled nursing facility (SNF) in Philadelphia, Pennsylvania, focusing on improving care for some of the most vulnerable populations. You will collaborate with a dedicated interdisciplinary team and lead initiatives that significantly impact patient health and quality of life. Key Responsibilities Conduct primary care rounds in skilled nursing facilities Manage chronic illnesses, diagnose medical conditions, and implement care plans Work with clinicians, facility leaders, and others to coordinate care and services Establish relationships with patients, their families, and facility staff to ensure effective treatment Provide oversight, mentorship, and guidance to nurse practitioners Perform clinical case reviews as necessary and share relevant tools, information, and guidelines to enhance care quality Represent our organization externally, fostering and maintaining relationships within the post-acute care community Collaborate with leadership to assess care coordination initiatives, develop intervention plans, and discuss network management strategies Qualifications At least 1 year of experience with a focus on delivering high-quality, patient-centered care Experience in post-acute care is advantageous but not mandatory, as we offer extensive support and development opportunities Possession of an active and unrestricted Pennsylvania medical license (or in the process of obtaining one) Active and unrestricted DEA/prescriptive authority (or in the process of obtaining one) A strong commitment to enhancing patient outcomes, promoting clinical excellence, and driving improvements in healthcare Candidates with backgrounds in internal medicine, family medicine, emergency medicine, hospitalist roles, or related fields are encouraged to apply Compensation The anticipated salary for this position ranges from $260,000 to $310,000 annually, with no limits on productivity-related income.
