organization that coordinates public health grants for a 26-county
region in central PA, seeking a Community Health Worker (CHW). This
is a full-time hybrid position located within our Carlisle Tapestry
of Health location, balanced with opportunities for work-from-home
days. The Community Health Worker (CHW) will provide individualized
health education and support to clients with lower incomes in their
communities. CHW will work collaboratively with agency staff and
partners to identify needs and connect clients to healthcare and
social services. Specific duties include making referrals to social
services, coordinating appointments, evaluating eligibility for
health insurance, collecting client data, evaluating participants
for various interventions such as Women's Health Services, SNAP
(Supplemental Nutrition Assistance Program), and WIC (Women,
Infants and Children). CHW candidate will also have working
knowledge of (or be willing to gain competency around) working with
individuals living with substance use disorder and active mental
health concerns. In this enhanced role, the CHW will be supporting
maternal and reproductive health initiatives, with a focus on
reducing disparities and improving outcomes for pregnant and
postpartum individuals. A High School Diploma or GED (General
Education Diploma) is required. Prior CHW experience or related
experience is a plus. CHW Certification will be required; training
will be provided upon hire. The successful candidate will also
possess strong verbal and written communication skills, time
management, and prioritization; computer literacy including
Microsoft 365; and the ability to work with cross-disciplinary
teams of diverse individuals, including those with lived
experience. Reliable transportation and a valid PA driver's license
are required for travel within the specified area. Child Protection

condition of continued employment: PA State Police Criminal Record
Check (PATCH) FBI Criminal Record Check, from fingerprinting PA
