Job Description:
ChristianaCare is seeking a dedicated Social Worker II to join
our dynamic social services team within our extensive health system
focused on providing exceptional healthcare across Delaware. In
this role, you will assist patients and their families in coping
with the psychosocial challenges associated with acute, chronic,
and terminal illnesses. Services entail psychosocial assessments,
diagnosis, treatment, and follow-up. This role requires a
compassionate approach and a strong understanding of healthcare
practices, contributing to our mission of delivering high-quality,
cost-effective care in the communities we serve. Conduct thorough
psychosocial assessments and formulate treatment plans. Collaborate
with healthcare team to provide comprehensive patient care.
Advocate for patients’ and families’ needs and resources. Provide
crisis intervention and support in decision-making processes.
Participate in multidisciplinary team meetings to ensure cohesive
patient care. Document and maintain patient records according to
legal and ethical standards. Strong interpersonal communication

Ability to manage stress Familiarity with healthcare software

401k Must be a Licensed Clinical Social Worker (LCSW) in the state
of Delaware or eligible for licensure. Certification in clinical
practice or healthcare social work is preferred.
