Job Description:

Laurel Manor Healthcare is seeking a dedicated and compassionate Social Worker to join our skilled Social Services team. In this role, you will focus on supporting the psychological and social well-being of our patients, including individuals undergoing rehabilitation and those in long-term care. As a pivotal part of our healthcare services, you will collaborate with medical professionals to develop tailored care plans and ensure the emotional and social requirements of our patients are met, aligning with our commitment to provide comprehensive healthcare services in a nurturing environment. Assess patients’ social and emotional needs, along with any economic barriers to health and wellbeing. Develop and implement comprehensive care plans in collaboration with healthcare professionals. Provide counseling and support to patients and their families during treatment and recovery. Coordinate with community resources to facilitate access to needed services. Document and maintain accurate records of patients' treatment and progress. Advocate for patients’ rights and best interests within the healthcare structure. Strong interpersonal and communication skills Ability to handle sensitive situations with empathy and discretion Proficient in case management software and medical records analysis Strong organizational and multitasking skills Medical Insurance Dental Insurance Vision Insurance Licensed Clinical Social Worker (LCSW) or eligible for licensure CPR certification preferred
