Job Description The Opportunity Are you an experienced
outpatient primary care Nurse Practitioner or Physician Assistant
who thrives in independent practice but is tired of the corporate

APP to join our growing, multidisciplinary care team. We practice

eligible for a pay differential. What You’ll Do Join a
mission-driven team that values meaningful patient relationships,
innovation, and provider well-being: True Independent Practice:
Manage your panel with autonomy. This role requires a provider who
is confident in their clinical decision-making and does not require
daily oversight. You must be comfortable with intermittent chart
reviews and academic discussion. Unhurried Care: See a manageable
16–19 patients per 8-10-hour day with standard 30-minute
appointment blocks. Comprehensive Scope: Diagnose and treat a wide
range of chronic and acute conditions across the lifespan (ages 5,
though we will consider adult-only for the right candidate).
Deliver direct patient care including preventative exams, urgent
care, and follow-ups. Integrated Care Model: Foster meaningful
relationships with patients and collaborate effectively with our
in-house behavioral health staff, MD/DO/ND providers, and
psychiatric providers. (Experience with mental health and
high-acuity populations is a massive plus!) Modern Workflows: Use
our Athena EHR with FreedAI Scribe to document encounters
accurately and efficiently. Minimal on-call duties, fully supported
by an after-hours answering service. Qualifications Licensure:
Licensed Family Nurse Practitioner (FNP) or Physician Assistant
(PA) in Oregon (or eligible to obtain within 90 days). Experience:
Strictly 2 years of outpatient, independent practice experience
required (outside of your education/clinical rotations). Clinical
Nuance: Highly comfortable managing complex, high-acuity
populations with comprehensive primary care. Skills: Strong time
management, culturally competent care delivery, and EMR
