This Home Health Physical Therapist Assistant position is based in
Forest Hill, Maryland, a welcoming Harford County community that

entertainment.As a Home Health PTA in this region, you will provide
one-on-one rehabilitative care to patients in the comfort of their
homes throughout Forest Hill and the surrounding Harford County
communities. You will work under the direction and supervision of a
licensed Physical Therapist, carrying out the established plan of
care and helping patients restore function, improve mobility, and
maximize independence. Your typical day will involve traveling
between patient residences, completing scheduled visits,
documenting care in an electronic medical record system, and
communicating regularly with the interdisciplinary team to ensure
continuity and quality of care.Typical responsibilities include
performing therapeutic exercises, balance and gait training,
transfer training, and functional mobility activities; instructing
patients and caregivers in home exercise programs; making
recommendations for home safety modifications and durable medical
equipment; and monitoring patients responses to therapy, reporting
changes to the supervising Physical Therapist. You will see a
diverse caseload, such as patients recovering from joint
replacement, orthopedic injuries, stroke and other neurologic
conditions, cardiac events, and general deconditioning. The home
setting allows you to tailor interventions to each patients unique
environment and goals, making your work highly impactful and
personally meaningful.Patient volumes and visit frequencies are
designed to support safe, effective care while allowing adequate
time for thorough treatment and documentation. Most PTAs in this
setting manage a field schedule that may include a mix of morning,
afternoon, and early evening visits, with flexibility to align with
patient needs and team scheduling. Weekday schedules are typical,
with potential for occasional weekend or holiday coverage depending
on agency needs and your availability.You will be part of a
supportive, collaborative clinical team that values communication,
professional growth, and evidence-based practice. Newer home health
PTAs can expect guidance from experienced clinicians, including
joint visits, case conferences, and ongoing feedback to help you
