Job Description:

• Provide Over the Top Care -
  
• Work as a Team to Accomplish Resident Goals
  
• Communicate with the Personal Care Home Administrator
  
• Communicate with Resident's Family Members
  
• Pass Medications
  
• Transcribe Written Doctors Orders on MAR
  
• Fax Medication Refills to Pharmacy
  
• Document/Chart on Residents
  
• Assist Personal Care Aides as Needed
  
• Follow Facility Policies, DHS Regulations, and HIPAA Law

• Every Other Holiday
